the following documents must be PRINTED AND submitted to the athletic director before the first day of tryouts/practice. **COACHES CANNOT COLLECT PAPERWORK** When submitting paperwork please make sure it is in the order listed below.
Emergency Contact Clearance Form must be turned in for each season/each sport.
Physicals are good for one calendar year.
Informed Consent COVID Form only needs to be turned in once per school year [with the first sport students participate in.]
