Transcript Information

** Transcript and Work Permit requests during the Summer from late June through August - should be directed to the District Transcripts & Work Permits (Summer)

** During Regular School year:

To request a copy of your transcript, please follow the steps below.

  •  See below to get the form (it is a fillable PDF)
  • Required information:
    • Name at the time of graduation (please include maiden name if applicable)
    • Graduation Year
    • Birthdate
    • Contact Phone Number
  • Fill out form and send to Washington High School NOTE:  Transcript Request for
    • 38442 Fremont Blvd. Fremont, Ca 94536
  • Include the following items:
    • An envelope stamped and addressed to where you want it sent (we will provide the return address information)
    • If you are requesting more than 1 transcript, please provide an envelope for each transcript
    • A check or money order for $5.00 per transcript

NOTE: The form may also be brought to the school office between the hours of 7:30 a.m. to 3:00 p.m.
